Harbor serves over 24,000 youth, adults, seniors and families annually and provides a full range of mental health, telehealth, and substance use disorder treatment services. Launch of OhioRISE

Ohio Department of Medicaid announced the launch of OhioRISE, Ohio’s first ever integrated program to help children who have complex and serious behavioral health needs. OhioRISE (Resilience through Integrated Systems and Excellence) will consist of intensive care coordination and new behavioral services. The program was jointly developed and will be governed by the Ohio Department of Medicaid, the Governor’s Office of Children’s Initiatives, and Aetna Better Health of Ohio who was selected to serve as the statewide OhioRISE Managed Care Plan (OhioRISE Plan). 

Harbor is proud to announce we have been selected as the care management entity (CME) that will provide moderate and intensive care coordination services for OhioRISE. With our extensive experience developing unique and complex programs to effectively meet the changing needs in our community, we will continue to be a driving force in addressing health equity through our partnership with Aetna. 

Harbor will serve as the care management entity for children, youth, and their families in Catchment Area B which includes these 11 counties – Wood, Ottawa, Erie, Sandusky, Seneca, Wyandot, Hancock, Huron, Crawford, Marion, and Union. 

Youth Care Coordination Services 

Looking for individuals with experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice, child welfare, or related field providing community-based services to youth and their families or caregivers.

Who will:

1. Effectively engage youth with significant behavioral health needs and their family.

2. Facilitates Child and Family Team meetings and ensure the child and family have a voice and choices in their plan and providers. Encourage creative solutions and use of natural supports with a goal of maintaining the youth in the community and in the least restrictive environment.

3. Provide support and assist youth and families in increasing social support skills and networks, coordinate and monitor all services identified in the Child and Family-Centered Care Plan, and provide assistance in gaining access to essential community resources.

4. Care Coordinator staff will have a small assigned caseload and ensure services are delivered using the High Fidelity Wraparound model.
